Interacting Services

Ohio Department of Aging gives Pleasantview an A-plus in resident satisfaction. Pleasantview Care Center has been rated significantly higher than the Ohio statewide average in recent resident satisfaction reports. These rating were determined through interviews with our residents, by independent agencies and by the Ohio Department of Aging.
Pleasantview Care Center offers a wide range of interacting services all designed to help coordinate the special needs of each of our residents.

General Services

  • Gourmet dining. Our specially trained kitchen staff prepares three delicious meals a day. Alternate choices are always available and in addition to our standard entrees, Pleasantview chefs often include popular ethnic choices.
  • Special Diets. Need a special diet? No problem, our highly trained chefs can make even the most restrictive diet yummy and nutritious.
  • 24 hour snacks. A variety of wholesome snacks are available at any time.
  • Beauty and barber services. To keep our residents looking beautiful (and handsome) we have a beauty shop and barber shop on site.
  • Eye, dental, and other services. No need to go out into the cold, Pleasantview offers dentistry, optometry, podiatry, x-ray, lab and pharmacy services on site.
  • 24 hour RNs on site. At Pleasantview, there is a full time nurse practitioner on site 5 days a week, and RNs on site 24 hours. a day, 7 days a week.
  • Physicians are always on call.
  • Psychological Services. We offer psychological and psychiatric support services and our staff psychiatrist visits residents on-site weekly.
  • Social services. We have full time, licensed social workers on site daily, who provide a wide range of services including discharge planning, answering questions on medicare, medicaid, hospice, and many other social services.

    Personal Assistance

    At Pleasantview Care Center, we realize that each resident is an individual with unique needs and preferences. To meet these needs, we meet with each resident and their family to conduct an assessment of the types of personal assistance they will need. These needs commonly include:
    • Personal hygiene and grooming
    • Dressing and clothes selection
    • Medication management
    • Continence management
    • Showering and bathing
    • Socializing with others and participating in activities
    • Help walking or otherwise getting around
    • Help transferring from bed, wheelchair, walker, chair, etc.

    Medical Services

    The highly trained staff at Pleasantview Care Center allows us to deliver many of the services normally available only in a hospital. Our 24 hour registered nursing coverage assures immediate, highly skilled, emergency care no matter what the time or day.

    Hospice care update:
    Of interest to our Catholic Residents, Crossroads Hospice (one of the hospices we use at Pleasantview Care Center), has hired a new Catholic chaplain - Sister Donna. For those that seek solace within their Catholic faith, Sister Donna is a very welcome addition.

    Pleasantview is committed to providing the highest level of care possible to those needing rehabilitation services through Therapy in Motion LLC, a local, therapist owned and operated, Medicare/Medicaid certified therapy company. They are a leading provider of rehabilitative services in Ohio with over 20 years of combined experience.

    Therapy in Motion allows us to provide Rehabilitation Services after discharge

    Because Therapy in Motion is a separate company, they are able to follow residents home after they are discharged. Many residents choose to work with the same therapist they have been working with at Pleasantview.

    More about our Therapy Services

    • We provide evaluations, treatment and education for individuals with a wide range of disorders including neurological disorders, post-surgical procedures, chronic pain syndromes, musculoskeletal injuries, work related injuries and speech and swallowing difficulties.
    • Each rehabilitation program is customized to meet the individual's specific needs.
    • For those at risk for falls and injury, we offer a therapeutic program focusing on posture, strength, balance and coordination to help reduce the likelihood of falling and serious injury.
    • To keep therapy fun, we employ a variety of games and group activities including ball games, chair exercises and Wii.
    • We administer modified barium swallows on site.
    • In order to ensure safe going home instructions, we provide home evaluations to identify potential problem areas.

    Our Pleasantview VA coordinators are experts at helping our residents coordinate VA services such as transportation, special care needs, doctor appointments etc.

    In addition to the Wade Park, Louis Stokes VA Medical center, veterans can also receive services at the conveniently located Parma Veterans facility.

    Help for veterans applying for the Aid and Assistance Benefit Program.
    If it is found that you or your spouse are medically qualified to live at Pleasantview Care Center or Legacy Place Parma, you may be eligible for a VA Aid and Assistance pension. While we can not help you with filing for benefits, our experienced VA coordinators can point you to a number of community resources and experts for in depth information and assistance.
    We are a family owned and operated Healthcare Center offering skilled nursing care, rehabilitation, intermediate care, hospice, respite and assisted living.

    At Pleasantview Care Center we offer a complete range of rehabilitation, I-V therapies, TPN, peritoneal dialysis and wound care services. Our assisted living home, Legacy Place Parma, offers full service accommodations for independent seniors as well as those needing personalized nursing care.

    How do I get started with my VA benefits?
    Nursing Home Care
    To determine if you qualify for VA benefits and you are currently admitted as a patient in a hospital, please discuss your VA benefit eligibility with your hospital social worker or case manager.
    Assisted Living
    Legacy Place, our assisted living home features 61 apartments with full service accommodations. Included in your room and board charges are housekeeping, laundry done on-site, three meals served daily in our dining room, activities and outings, as well as transportation to doctor appointments within our service area. Telephone, utilities and property taxes are also included in the room and board rate. Should you need assistance with nursing or personal care, additional charges would be made.

    As a Veteran you may be entitled to an Aid and Attendance benefit through your VA pension benefit. A spouse may also be eligible for this benefit. We will be happy to refer you to sources in the community who can help you apply for these benefits.     Social Services

    At Pleasantview, our full time, licensed Social Workers and other social services staff members are always available to help residents and members of our community in any way they can. Below are a few of the services we provide:
    • Medicaid Liaison. Are you eligible for medicaid? What are the requirements? Do you need help filling out the forms? Pleasantview social workers will help you locate the documentation you need, and help you fill out the paperwork.
    • Help filling out other medical and legal forms. Do you have a signed Power of Attorney? A Do Not Resuscitate (DNR) form? An Advanced Directive? If you do not, we can help you fill out the forms and have them notarized if necessary.
    • Help with providing discharge planning to home or to Legacy Place Parma, our assisted living facility. Many of our short term rehab residents choose to move to our assisted living after they have completed rehab.     • Wheelchair accessible. All of our areas are wheelchair accessible, and residents are encouraged to move about, visit with friends or participate in our many activities.
    • Outdoor patio. Our lovely outdoor patio is perfect for sharing time with visitors, planting flowers, or just enjoying the waterfall and listening to the birds sing.
    • Comfortable, familiar living environment. We encourage residents to bring their favorite lounge chair, dresser, photos or other personal items with them.
    • Emergency Call service. Help at Pleasantview Care Center is always just a minute away.
    • State of the art security systems. All areas have security, smoke alarm, fire safety and sprinkler systems.
    • Smoking area. For residents that smoke, we offer a comfortable, designated smoking area.
    • Many room and apartment choices. Accommodations include newly remodeled private and semi-private rooms with private bathrooms. Many include showers with pocket doors.

    Activities

    The activities program at Pleasantview was designed to help our residents feel happy and comfortable in their new environment. Our activities department makes sure there are interesting things to do everyday - including weekends.

    Pleasantview offers a diverse and stimulating range of activities designed to enhance and encourage each resident’s individual independence and enjoyment of life. Below are a few of our most popular activities.
    • Art therapy
    • Music therapy
    • Bingo, cards, board games
    • Weekly entertainment and socials
    • Religious services - Catholic Mass and rosary, gospel hour, prayer groups, etc.
    • Learning and recreational activities based on residents interests
      • Exercise routines specifically designed for joint and arthritis pain
      • Visits by organizations within the community
      • Intergenerational programs - Parma schools, girl scouts, boy scouts, etc.
      • Lunch bunch - a group of residents that gets together once a month to plan and prepare a favorite meal.
